Licensing Program Analyst (LPA)Tiffany Holmes contacted the facility to deliver findings for a complaint investigation via tele-virtual. LPA identified herself to, and explained the purpose of the visit and the basic elements of the complaint with Alexis Garcia, Program Director.

Licensing Program Analyst (LPA) Domingo previously conducted a tour, made observations, and obtained and reviewed pertinent records on 04/28/2025. LPA Holmes conducted interviews with clients, staff, and outside sources. It was alleged that lack of staff supervision resulting in client on client altercation occurred. Interviews revealed Client 1 (C1) had been struck by C2 on the head. Interviews revealed that C2 was immediately redirected. Interviews revealed the staff provided C1with an ice pack for their head. Interviews revealed that staff completed a SOC 341 and reported the incident to Community Care Licensing (CCL) and San Diego Regional Center (SDRC).

Interviews with outside sources revealed that C1 was also hit by C3. Interviews revealed that neither C1, C2 and C3 are at this program anymore. Interviews with outside sources revealed the clients are being supervised. Interviews with staff revealed clients are supervised and when an incident occurs the other client is always redirected.

It was alleged that the licensee did not provide incontinence care. Interviews revealed staff change clients when they are wet and soiled. Interviews revealed that the program provides incontinence care to include the staff asking clients do they have to go to the bathroom, reminders about the bathroom and being changed every hour. Interviews revealed that the staff working with the client is the one in charge of incontinence care. Interviews revealed that they kept a log for C1 regarding their bathroom uses, changes of briefs and accidents. Interviews revealed the staff are providing incontinence care to the clients in care.

It was alleged that staff neglect and lack of supervision. Interviews revealed that it was alleged that staff found C1 sitting alone in their soiled undergarments that also soiled the outer clothing. Interviews revealed that it's difficult to get C1 to use the bathroom because they are non compliant and refuses to go to the bathroom. Interviews revealed that C1 is not left alone and that they are often to themselves due to not wanting to deal with other clients. Outside source interviews revealed there was enough supervision. Outside source interviews revealed that everytime they went to the program to see about C1 that they were being supervised and cared for. Interviews revealed that the clients are always with a staff. Interviews revealed there are 3 clients with 1 staff while out in the community and inside program. Interviews revealed the staff do not neglect clients and provide supervision to clients in care.

The Department has investigated the above-mentioned allegations and based on interviews, LPA observations, and records review, it was determined that the complaint allegations are unsubstantiated. The allegations may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations occurred.

An exit interview was conducted with Alexis "Lexie" Garcia, Program Director via face time and a copy of this report along with Licensee/Appeal Rights (LIC 9058 03/22) was provided via email. An electronic email read receipt confirms the documents were received.
