Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced investigation visit to open a complaint investigation regarding the above mentioned allegations. LPA was greeted by, identified herself to, and explained the purpose of the visit and the basic elements of the complaint with Nick Fierro, Administrator.

During the visit, LPA toured the facility, reviewed and obtained copies of facility records. LPA conducted interviews with staff and clients. It was alleged that the staff does not provide residents a safe environment. Interviews with clients revealed they feel safe at the facility and know they can speak to staff if anything ever makes them feel unsafe. Interviews revealed that there are clients that argue at times and it is expected because of how many people are at the facility. Interviews with staff revealed they make the clients feel at home and always promote safety. The staff conduct rounds to make sure there is nothing going on in the rooms, make sure strangers are not in the facility and lastly make sure that the clients are not doing anything that could possibly cause them harm. Interviews did not reveal that clients did not feel safe or that the staff are there to asssist them. Interviews with staff stated when there are verbal altercations they let the clients work it out and if they can't figure it out on their own they will step in and de-escalate the verbal altercation and divert the clients attention somewhere else. Interviews revealed most clients do really well with redirection. Based on interviews, the allegation of staff does not provide residents a safe environment is Unsubstantiated. An exit interview was conducted with Nick Fierro Administrator. A copy of this report and the Licensee's Rights (LIC9058 03/22) were provided to Administrator at the end of the visit.
