On date and time listed, Licensing Program Analysts (LPAs) Sumayya Habeebulla and Kelli Waters arrived at the facility to conduct an annual visit for the facility. During the visit, LPA Waters was able to verify the proof of correction from a previous deficiency dated 05/31/24. LPAs completed a tour of the facility with Director, Valerie Rodriguez, proof of corrections were made and POC letter was printed at time of visit.

LPAs observed the cement area in the 2-year-olds playground had been repaired and all the uneven ground was corrected.

Based on observations made during today’s visit, the plan of correction has been met. An exit interview was conducted, and a copy of this report was provided to the facility director.

Notice of Site visit must be posted for 30 days.
