On 06/19/24, at time listed above, Licensing Program Analysts (LPA), Kelli Waters and Sumayya Habeebulla arrived unannounced and conducted an annual inspection as part of a compliance review. This is a combination childcare center, the other licensed program is an infant program, which was no inspected on this date. LPAs met with Interim Director Jerona Nguyen. A tour of the inside and outside of the facility, Building 1 and 2, was granted and the following was observed and/or noted:

• The following items were posted and updated where necessary:
- License
- Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148)
- Parent’s Rights Poster (PUB393)
- Personal Rights (LIC613A)
- Child Car Seat Law
- Menu
• The facility is operating within the limits as stated on the license.
• Ratios are being met during this inspection; 5 staff and 22 children present
• Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards
• There are no weapons present at the facility as stated by Facility Representative, Nguyen.
• There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
• Drinking water is provided in the indoor activity space by personal water bottles brought from home or school water cooler - and in the outdoor activity space by personal water bottles or water cooler
• Medications are stored where inaccessible to children in locked upper cabinets
• Hazardous items are stored where inaccessible to children which include disinfectants, cleaning solutions and other items that are dangerous
• Poisons and toxins are locked and inaccessible to children
• All floors were observed to be safe and clean.
• Bathrooms were observed to be safe, sanitary and in operating condition
• Playgrounds are enclosed by appropriate fences; tripping hazards were present. See deficiency 809-D
• Outdoor activity areas are supplied with age and size appropriate equipment
• Food preparation area is clean, free of litter and rubbish and free of rodents and other vermin

Food is stored appropriately
• All storage containers for solid waste were observed to have tight-fitting covers that are kept on, and in good repair
• Sign in/Sign out record was reviewed and meets regulation requirements
• Disaster drills are conducted at least every six months – last drill was conducted on 02/16/24
A review of staff and children's records were conducted as part of this evaluation.
• Children’s records were found to be incomplete during this inspection.
• Staff record review indicates that not all staff present had documentation of meeting minimum qualifications for the position for which they were hired; transcripts will be provided when acquired.
• A staff member is present with current Pediatric CPR/First Aid which expires on 02/25.
• Opening and closing staff member’s CPR/First Aid expires on 02/25
• Director completed Health and Safety Training
• A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP)

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
