25 | On April 14, 2022 at 12:46 PM, Licensing Program Analyst (LPA) Cindy Hamilton arrived at the facility to conduct an unannounced case management visit on an Unusual Incident Report (UIR) dated 03/24/2022. The UIR was regarding a child stating the child was pinched by a teacher. LPA met with Learning Jungle Escondido (CCC) Paige Eusebio, Designated Facilitator to discuss the incident.
On 03/24/2022, it was reported that on 03/22/2022, Ms. Eusebio became aware that a staff member had allegedly pinched a child on the tummy and was being mean. On 03/23/2022, Ms Eusebio spoke with the staff allegedly involved about the allegation and staff denied it. During this visit, LPA conducted interviews and was unable to corroborate the allegation.
No deficiencies are being cited at this time as there appears to be no violations of Title 22 Regulations. The facility immediately took appropriate action and staff allegedly involved has resigned. The CCC was advised to conduct personal rights training with all staff to ensure safety and wellbeing of children in care.
An exit interview was conducted, appeal rights discussed and a copy of this report was provided to Designated Facilitator Paige Eusebio.
A Notice of Site Visit was given and Ms. Eusebio was reminded it must remain posted for 30 days. |
