25 | On the above date and time, Licensing Program Analyst (LPA) Kelly Gerth arrived at the facility on a case management inspection, to follow-up on an Unusual Incident Report (UIR) called in to CCL Duty Line on January 17, 2025.
The UIR stated on January 17, 2025 a child enrolled in the facility was exhibiting challenging behaviors that included attempting to bite other children and hitting other children and staff. The facility stated they would be implementing a behavior plan. During the visit, Site Supervisor Yadira Lopez detailed that the facility has attempted to schedule 3 meetings with the parent but has not been successful, to date, due to the parent’s lack of cooperation. In addition, the Child Childcare Center (CCC) has provided the parent with resource and referral information for both parent and child, however the parent has yet to follow through with any of the referrals or resources provided.
During a joint conversation with Site Supervisor Yadira Lopez and Program Director G. Espinosa, it was revealed that within the agency there has been limited support above the site level to onsite center staff, the child and parent. LPA provided consultation for consideration of additional site level support from Program Director.
Based on the information obtained during the visit, there appears to be no violations of Title 22 Regulations pertaining to the reported incident and LPA determined that the facility took the necessary steps to ensure the safety of the children, at this time.
An exit interview was conducted and a copy of this report was provided. Notice of site visit was provided and must remain posted for 30 days. |
