On November 8, 2023 at 1:15 p.m. Licensing Program Analyst (LPA) Leilani Curtis conducted an unannounced inspection to follow up on a self-reported incident that occurred on 10/30/23, wherein a staff member (S1) was terminated for violating the personal rights of several children in care. Upon arrival LPA met with Licensee/Director Karyn Begin and toured the facility. There were 41 napping children with 6 staff members present. Appropriate ratio/capacity was observed. Staff members have the required background clearances and are associated to the facility.
LPA interviewed Licensee Begin, staff #2 (S2), staff #3 (S3) and reviewed facility records. The licensee states that due to concerns brought to her by a parent on 10/25/23 and a staff member (S2) she began viewing surveillance footage of the classrooms. The licensee states that she observed the following actions by S1:
9/29/23 at 12:33 p.m. S1 left child #1 (C1) standing alone on top of the changing table. S1 was observed yelling at C1 and pushing the child’s leg causing her to lose her balance. S1 used foul language in front of the class.
10/24/23 at 11:24 a.m. S1 yells at child #2 (C2) causing another child to scold C2 and place C2 on a chair. S1 films the interaction and laughs.
10/25/23 at 11:01 a.m. child #2 (C2) was dancing and fell onto child#3 (C3). S1 comforted C3 and slapped C2 in the face.
10/25/23 at 2:32 p.m. S1 uses her foot to reposition the napping mat of C2 while he is on it. S1 directs C2 to stand and walk on S1’s back. S1 lays on C2’s mat leaving C2 to sit on the floor next to his mat.
10/26/23 at 8:33 a.m. child #4 (C4) softly hit S1. S1 hit C4 twice, yelled at C4 and pushed C4. S1 laughs at C4.
The licensee states that the parents of the children involved were notified of the incidents and S1 was terminated on 10/30/23.
