13 | On 11/6/25, Licensing Program Analyst (LPA) Saraliz Velando conducted an unannounced visit to deliver findings for a complaint received on 8/22/25. The LPA met with the Director, Karyne Begin. LPA met the Director at an offsite location due to the facility undergoing repairs at this time and also in inactive status. Based on file review and staff interviews, there was not enough proof of the allegation that staff did not report child’s injury to an authorized representative. Based on staff interviews, there was also not enough evidence to prove that staff left children unattended in a dark room. The child was picked up during nap time and so the room was dark and a staff member that retrieved the child from the room was the only one that entered the napping room and so the person picking up the child could not have seen that there were other people in the classroom. Information from interviews show that there were other staff and children not visible from outside the classroom door. As for the allegation of staff did not follow proper sign in/out procedures, the facility has passcodes for persons authorized to pick up children and records showed the different codes that were used to pick up the child. While passcodes were present showing Child 1 was signed in and out, other parties were sharing codes, so it is unclear if Child 1 was signed in and out properly. Although the allegations may have happened or are valid, there is not enough evidence to prove that the alleged violations occurred, therefore the above allegations are found to be unsubstantiated. No deficiencies were cited today. |
