On July 13, 2021 at 9:50 a.m. Licensing Program Analyst (LPA), Leilani Curtis conducted an unannounced Annual Inspection and met with Licensee, Tanya Odeesh. Also present were the licensee’s parents Astefanous Gorgees and Maryam Gorgees. Language Link interpreter ID#12322 translated for the licensee. The licensee’s primary language is Arabic. There were no children present. LPA conducted a tour of the home inside and outside per facility sketch. Licensee is using the following areas for daycare: living room, dining area, kitchen, bedroom #1, bedroom #2, bathroom #1 (located within bedroom #1) and enclosed rear and side yard. Off-limits areas include: bedroom #3, bedroom #4, bathroom #2 and garage.

Licensee has provided adequate space for the children to eat, sleep and play within the home. Home is clean, orderly and has adequate ventilation. Children’s toys and play equipment are available and observed free of hazards. There are no stairs in the home. There is a working telephone/email address. There were cleaning compounds and medication accessible to children in bathroom #1 and knives and a lighter accessible to children in a kitchen drawer. There is no fireplace. The fire extinguisher and carbon monoxide detector are operational. The licensee does not have an operational smoke detector. Licensee states there are NO firearms or other weapons in the home. Outdoor play area is fenced. There are hazardous items in the outside yard including lighter fluid, a lighter, charcoal and barbecue tools which are accessible to children. There is a fire pit located in the side yard. The licensee states that the fire pit will not to be used when daycare children are present. There are no existing bodies of water present. Children records were reviewed for Emergency Information. The licensee does not have a roster or a completed disaster/fire drill log available for review. There are no new adults living or working in the home over the age of 18 years. All adult residents and helpers have submitted or been cleared for criminal record and child abuse index clearances or exemptions. Pediatric CPR and First-Aid certificates are valid through August 2022.

LPA reviewed the following with Licensee: Safe Sleep Regulation Concept Handout/PIN 20-24-CCP, Car Seat Law, emergency drills, effects of lead, SIDS, and Shaken Baby Syndrome. Licensee is reminded that corporal punishment, smoking, walkers, exersaucers, jumpers and bouncy seats shall never be permitted during daycare operation. Licensee is aware that interference with a child’s daily functions, corporal punishment, physical and mental abuse is not allowed. Licensee is reminded to make anything that reads, "Keep Out of Reach of Children" inaccessible to children. Licensee is exempt from Mandated Reporter AB1207 training certification due to applicant having limited English proficiency. Immunization records per SB792 were reviewed and are in compliance.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. See LIC809D for cited deficiencies. LPA reviewed this report with Licensee and provided a copy of appeal rights (LIC 9058 01/16). Her signature on this form acknowledges receipt of these rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed Licensee post notice of site visit.
