On January 8, 2026, at 2:47 p.m., Licensing Program Analyst (LPA) Evelyn Reyes conducted an unannounced complaint visit regarding the allegations received on October 27, 2025. LPA Reyes met with the Licensee, Maria De Mello Pacheco, identified herself, explained the purpose of the visit, and was granted entry into the facility. At the time of the visit, LPA observed eight children and three infants in care, supervised by the Licensee and one helper, Tamires Santos.

The allegations stated that the Licensee was operating out of ratio/capacity, using inappropriate discipline, and interfering with children while they were eating. During the investigation, LPA conducted interviews, made facility observations, and reviewed relevant documents. Continue on page 2.

At 2:54 p.m., all toddlers and one infant were observed sleeping, while two infants were awake and being supervised by staff. LPA observed that each child had their own cot, crib, or playpen for sleeping. At 4:03 p.m., LPA observed children being fed without any interference. No inappropriate discipline was observed during the visit. Therefore, the allegations of operating out of ratio/capacity, using inappropriate discipline and interfering with children eating were found to be Unsubstantiated.

An Unsubstantiated finding means that although the allegation may have occurred or may be valid, there is not a preponderance of evidence to show that a violation took place.

An exit interview was conducted with the Licensee. A Notice of Site Visit (LIC 9213) was issued and must remain posted for 30 days. Appeal Rights (LIC 9058) and a copy of the report (LIC 9099) were provided to the Licensee.

The exit interview was completed, and the report was reviewed with Licensee Maria De Mello Pacheco.
