On 2/25/22 at 3:30 PM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced POC (Proof of Corrections) visit with the Licensee, Azada Hamdard. Upon arrival, LPA Lane introduced herself and the reason for the visit. There were no daycare children present at time of visit. Also present at the facility were Licensee’s three minor children, adult helpers Arzo and Mohammad as well as her husband Ghulam who translated in Farsi.

At 3:45 PM, LPA observed car seats inside white Sienna van and additional car seats in the home. Licensee stated that she always has the children under the age of 8 years in car seats when transporting them. Licensee stated she understands that if the child is under 2 years old that the car seat should be rear-facing.

At 3:55 PM, LPA observed back yard fencing and saw there are places that have been recently fixed by Licensee. There were holes in the fence but they have been repaired. LPA Lane advised that the Licensee continues to maintain and observe the area on a weekly basis to ensure the children cannot get into the neighbor’s backyard who has a pool. LPA Lane pushed on the fencing to ensure it was secure.

Licensee stated they have 4 adult helpers and maintain visual supervision of the children at all times while outside.

No deficiencies were cited. LPA Lane provided clearance letter for previous citation regarding the car seats.

Exit interview conducted and report was reviewed with the licensee, Azada Hamdard. Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements is an immediate civil penalty of $100.
