On 3/17/25 at 1:00 pm Licensing Program Analyst (LPA) Gerald Poindexter conducted an unannounced annual inspection. Upon arrival, LPA met with licensees Yasmeen and Hamidullah Shahzad. Also, in the home were four of the licensees’ own children. Present in the home were 4 daycare children. The licensee was provided with the Inspection Checklist (LIC 126). The 3-bedroom, 2-bath home was toured and inspected to ensure an environment safe for the care and supervision of children.

The licensee has provided adequate space for the children to eat, sleep and play within the home. Areas used for child care include Living Room, Kitchen, Dining room and Restroom #1, Deck (leading to backyard) and Backyard. Off limit areas: Garage, Bedroom 1, Bedroom 2, Bedroom 3, and Bathroom #2. Off limits areas have been made inaccessible with the use of door knob covers and locks. There are no stairs in the home. The facility has sufficient toys and equipment available.

The home has a fully-fenced, backyard play area available for outdoor activities. LPA observed exposed nails and screws in several locations of the fence. Also, the latch/lock to the fence door leading to the off-limits garden area was broken. LPA observed a trampoline and advised licensee that manufacturers age requirements must be followed for all play structures. Licensee Mr. Shahzad stated that the structure is not used for daycare children, but only his own, older children. The licensee understands that visual supervision is always required during outdoor activities. No body of water was observed during time of inspection. However, LPA provided Assembly Bill (AB) 2866 (Pellerin), Chapter 745, Statutes of 2024 in order to comply with the pool safety requirements, effective January 1, 2025.

There is a fully charged fire extinguisher, smoke and carbon monoxide detector that meet requirements and are operational. There is no fireplace. Poisons, cleaning compounds, medications and other hazardous items are not made inaccessible to children (see LIC809D). Adequate heating and ventilation are provided and vents are located at the ceiling level. There is a working telephone and email address. Licensee stated there are NO firearms and weapons in the home.

LPA observed that all required postings were posted. Children’s records were reviewed and found to be incomplete. Staff records were reviewed. Licensees have the required immunizations per SB792. Licensees’ Pediatric CPR/First Aid are current with an expiration date of January 2026. Preventative health practices course was completed, which includes lead poison prevention training. Licensees are exempt from Mandated Reporter Training Certificates due to language. Their primary language is Pashto.

Emergency drills were conducted and documented. However, the last documented drill was 8/25/24, more than 6 months ago. Licensee maintains a current roster of the children which LPA obtained during time of inspection. LPA verified that all adults living or working in the home have been fingerprint cleared and associated. LPA reminded Licensee that all unusual incident reports shall be submitted to Licensing office via email at SDIncidentReports@dss.ca.gov or via fax at (619)767-2203. Duty officer number is (619) 767-2248.


Provider is hereby reminded of the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms; corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer. LPA and licensee discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov.

Licensees were reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA conducted child care quality management staff interview with the licensees. LPA discussed the safe sleep regulations with licensees and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensees of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensees were informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA Gerald Poindexter informed licensees Yasmeen and Hamidullah Shahzad that this report dated 3/17/25 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Gerald Poindexter informed the licensees Yasmeen and Hamidullah Shahzad to provide a copy of this licensing report dated 3/17/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with the licensees Yasmeen and Hamidullah Shahzad. During the exit interview, the licensees, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. A Notice of Site Visit was given and must remain posted for 30 days. Appeal rights provided.
