On 05/2/24 at 1:45 pm, Licensing Program Analyst (LPA) Annette Sutherland conducted an unannounced complaint inspection to deliver findings regarding the above allegations. LPA Sutherland met with Licensee Ann Ayoob Also present was licensee’s husband Tareq Alias and adult son Zeyad Zora who helped translate to Arabic.There were 4 children present. The Department fully investigated the above allegations. LPA observed adequate supervision being conducted by licensee and husband on several visits during the complaint investigation and did not witness any inappropriate behavior with the children. LPA has witnessed Licensee and husband change diapers while at the facility and verified that they change them in a timely manner. LPA interviewed children, parents, and potential witness and there were no corroborating reports of concerns about supervision, inappropriate behaviors, or diapers changing concerns. Although LPA did not find evidence of the allegation during the investigation, it cannot be conclusively

proven or disproven an incident regarding children interacting in an appropriate manner. Therefore, it is determined to be Unsubstantiated. A finding of Unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violation occurred. An exit interview was conducted with the Licensee. A Notice of Site Visit (LIC9213) and Appeal Rights (LIC9058) was provided along with the report (LIC9099) to the Licensee. LPA Sutherland observed Notice of Site Visit being posted. Notice of Site Visit must be posted for 30 days.
