On 2/7/25 at 11:20AM Licensing Program Analysts (LPAs) Annette Sutherland & JC Valdez conducted an unannounced annual inspection with the Licensee. Upon arrival, LPA met with Licensee Ann Ayoob also present was licensee’s husband Tareq Alias. Licensee was provided the Inspection Checklist (LIC 126). The 1 story, 3-bedroom, 2-bathroom home was toured and inspected to ensure an environment safe for the care and supervision of children. Present in the home were 6-day care children (including 2 infants). Later on in the day 1 child left and 3 new children arrived. Licensee has provided adequate space for the children to eat, sleep and play within the home. Areas used for childcare include kitchen, dining room, living room, bedroom #3, bathroom #1, and front yard for childcare. Off limits areas include bedroom #1, bedroom/bathroom #2, garage and backyard. and are inaccessible through use of doorknob covers and door locks.
The licensee has sufficient toys and equipment available but was reminded to bring them out during visit. The home has a fenced backyard that is currently not being used for children in care. Licensee uses front yard for outdoor activities. front yard is fenced . Licensee understands that supervision is always required during outdoor activities in the front yard as the home is on a main street.
The fire extinguisher, smoke detector, and carbon monoxide detector meet requirements and are operational. All hazardous items were not latched or locked. Liquid detergent under the kitchen sink was accessible and there was a drawer in the kitchen that consisted of sharp objectives accessible to children. There was also children medication on the counter and was also accessible to children. Licensee was reminded that all hazardous items need to be latched and locked and out of reach of children. Licensee had a small space heater on at arrival that also posed an immediate risk to children as it was in an on-limit area and accessible to children. There are no bodies of water on the property. Licensee states that there are no weapons in the home.
First Aid and CPR certifications expire in March 2026 for both licensee and husband. Licensee has required immunizations. Licensee is exempt from Mandated Reporter Training as their primary language is Arabic.

Licensee was unable to provide records for children present. Licesnee was able to later pull out the 3 files for 3 new children that arrived. LPA obtained an updated facility roster and observed required documents are posted in an area accessible to parent/guardians. Provider is hereby reminded of the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms; corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer. LPA and Licensee discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov. LPA reviewed with Licensee the LIC 311D, Forms/Records. To Keep in Your Family Childcare Homes, children’s forms/records, facility forms/records, and information to be posted. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Childcare Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.LPA discussed the safe sleep regulations with licensee and discussed the Childcare Licensing Safe Sleep web page at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Childcare Homes Section 102417. LPA Annette Sutherland informed licensee Ann Ayoob that this report dated 2/7/25 document(s) 2 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Annette Sutherland informed the licensee Ann Ayoob to provide a copy of this licensing report dated 2/7/25 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the licensee Ann Ayoob. During the exit interview, the licensee, Ann Ayoob, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.A notice of site visit was given and must remain posted for 30 days.
