On November 13, 2023 @ 10:17AM, Licensing Program Analysts (LPAs) Nancy Diaz and Sherlynn Banas conducted an unannounced inspection. The purpose of this inspection is to observe corrections to deficiencies cited on October 30, 2023. Licensee, Hadel Boules was home today with two children, but were under age two.

The following deficiencies are now corrected:

1. The converted garage is now inaccessible to day care children via lock. Mrs. Boules is aware that she must obtain proper permit to allow use of area for day care children.
2. Play equipment were moved to the front play yard. Children are no longer using the play yard next to the garage.
3. Vaccination document observed for helper Randa Yaqo.

No further deficiencies observed today.

Exit interview was conducted with licensee, Hadel Boules. LPA reviewed the report and provided a copy today. Appeal rights and Notice of site visit were given today.
