25 | Licensing Program Analyst (LPA), Tyra Block, conducted an announced Pre-Licensing inspection with the applicant. The single story, 4 bdrm, 2 ba home was toured and inspected to ensure an environment safe for the care and supervision of children. The fire extinguisher (3A40BC), carbon monoxide detector and smoke detector meet requirements and are operational. All poisons, cleaners, and hazardous items were latched/locked and secured out of reach of children, and electrical outlets were covered. There are no bodies of water in the home. Firearms in the home are locked and stored according to regulation. Applicant has certified on the application they have sufficient financial resources to sustain the license. Applicant owns the home and has provided proof of control of property. A review of records on this date indicates that all facility staff/ residents or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. First Aid and CPR expire on 4/24 and preventative health practices course was completed on 3/13/22. Mandated Reporter Training was completed on 1/17/22. TB clearance and immunization requirements were met. Influenza vaccine or declination is required annually September- December.
Applicant will be using the following rooms for childcare: Kitchen, Dining Area, Living Room, Bedrooms #2 and #3. The following areas will be off limits: Bedrooms#1 and #4, and side yard. They are made inaccessible with the use of door locks and locked gate. The applicant has age-appropriate toys and equipment available for children to use and a fenced backyard available for outdoor activities. Applicant understands visual supervision is to be maintained at all times during activities away from the facility. Applicant was advised no changes should be made to the home without prior notice and/or approval from Licensing.
LPA reviewed with applicant the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted.
The new provider packet was reviewed with the applicant including information on the effects of lead, child abuse reporting, roster and emergency drill log example, visual for ratio/capacity, immunizations, adults living or working in the home, shaken baby syndrome, SIDS, a Guide to Safe Sleep (Safe Sleep Concepts dated 4/2018 provided), and the YMCA Resource Center.
