On 5/7/24 Licensing Program Analyst (LPA) Gerald Poindexter conducted a case management visit for the purpose designating one repurposed “on-limits” room, bedroom #2/classroom #2 to the licensee. The facility is not requesting a capacity increase at this time. LPA met with licensee Lisett Valenzuela Ortega. Also, present was the licensee’s mother/helper and 6 daycare children observed by LPA during naptime. Licensee states that hours of operation Mon-Fri 7am top 5:30 pm..

The home was inspected to ensure an environment safe for the care and supervision of children. LPA conducted a tour of the home, per facility sketch. During the visit, LPA checked the new room for safety and age appropriateness. The room is designated as classroom #2 and contains toys, learning materials, and a video projector. The closet and electrical box are latched/locked. Floor vent is partially blocked by a bookcase.

The following areas are used for child care: Living room (classroom #1), kitchen, bathroom, bedroom#2/classroom #2, and back deck (for entry only) and front patio area. Off limit areas continue to include: the bedroom #1 next to the restroom and the outdoor storage area. These areas are made inaccessible to day care children using doorknob covers and locks. There are latches on all cabinets in the kitchen and bathroom. The licensee has sufficient toys and equipment available.

The smoke detector, and carbon monoxide detector meet requirements and are operational. The fire extinguisher was observed to meet requirements. Hazardous items were secured out of reach of children with latches/locks or high placement. There is no body of water on the property. Licensee states that there are no weapons in the home.

Pediatric First Aid and CPR certifications for licensee expire on 8/2025. LPA advised licensee to have her helper obtain Pediatric First Aid and CPR certifications, as backup in case licensee could not perform emergency care. Licensee and helper have required immunizations. Licensee and helper completed Mandated Reporter Training on 6/14/22 and 10/22/22, respectively, and are reminded it must be completed every 2 years.

LPA observed all required postings were posted. Children’s records were reviewed and found to be in order. Staff records were reviewed. Licensee maintains a current roster of the children which LPA obtained during time of inspection. LPA advised that roster must be regularly updated and maintained for period of three years.

Emergency drills were conducted with the most recent being 1/12/24 and must be conducted and documented every six months. LPA verified that all adults living or working in the home have been fingerprint cleared and associated. LPA reminded Licensee that all unusual incident reports shall be submitted to Licensing office via email at SDIncidentReports@dss.ca.gov or via fax at (619)767-2203. Duty officer number is (619)767-2248.

Provider is hereby reminded of the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms; corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

During the visit, LPA inspected the repurposed room and approved it for child care and added to license. Exit interview conducted and report was reviewed with the facility representative, Lisett Valuenzuela Ortega. A notice of site visit was given to license and must remain posted for 30 days.
