25 | On November 21, 2023 at 1:06 PM., Licensing Program Analyst (LPA), Sherlynn Banas met with Licensee, Fatimah Zakarya and Sarah Alomar (daughter) for the purpose of increase in capacity from eight (8) to fourteen (14) children. There were no children in the care at this time. .
LPA toured the home. The licensee owns the home. There are 4 bedrooms and 3 bathroom in the house. All required forms were posted. There is a working telephone on the premises. There are no bodies of water, however there is a fountain which doesn't have water. LPA, Jennifer Lott advised licensee to put flowers in the fountain. There were no weapons/firearms or ammunition in the home or on the property. The fire extinguisher size (5MB-6HA) meets requirements and is fully charged, located in the kitchen. The dual smoke detector and carbon monoxide detector is mounted on the hallway ceiling and is operational. Applicant’s Pediatric CPR/FA certification is valid through June 2025. The Fire Safety Inspection Request (STD 850) was approved by the local fire marshal and GRANTED the capacity increase to fourteen (14) children on November 17, 2023. The fireplace was secured and inaccessible to children.
Applicant will be using the following areas for childcare: Living room, dining room, kitchen, family room, guest room, and backyard. Off limits areas of the home were all the second floor rooms, laundry room downstairs, and the garage.

Outdoor play area is fenced and equipped with age-appropriate play equipment and toys, in good condition. Applicant was reminded of requirements for children’s records, child abuse reporting, unusual incident reporting, immunization, criminal background clearance procedures and policies, and posting requirements.
LPA reviewed the Fire Drill; the last fire drill was August 3, 2023.
