On 08/01/2023 at 8:50am, Licensing Program Analyst (LPA) Selina Siao conducted a follow up pre licensing inspection to ensure that the required corrections listed on the pre licensing report dated 07/24/2023 have been corrected. Present at the home today is applicant Gabriela Cossich, her partner James Dinneen and their daughter.

The following corrections have been corrected:
  • A facility sketch for the upstairs off limit area was submitted by applicant
  • The fire pit has been moved to an off limit area
  • The BBQ grill located at the side of the yard has a cover
  • The cabinet with glass located at the dining area has been latched
  • The two kitchen drawers with hazardous and sharp items have been latched
  • Bottom of the garage door has been covered to ensure safety of the children
  • LPA confirmed that the weapons located in the safe and the ammunition are stored separately.
  • The small trampoline located at the backyard is for children ages 6 and older. Only one child should be inside at a time. Applicant understands that the trampoline should be inaccessible to children younger than 6 year old.

The home appears to be within substantial compliance and the fire clearance for a large license was granted on 07/17/2023. Applicant is registered for the 8 hours preventative health practice course that will began on 09/05/23 ending on 09/14/23. A provisional license will granted today and it is valid for 90 days.
