25 | On October 9, 2024, at 12:45 PM., Licensing Program Analyst (LPA), Sherlynn Banas met with Licensee, Liana Ayrapetyan for the purpose of increase in capacity from six (6) to twelve (12) children. Licensee’s helper. Ana Gazarova is also present. There were 6 daycare children. Licensee stated there are 5 enrolled children.
LPA toured the home. There are five bedrooms and three bathrooms. All required forms were posted. There is a working telephone on the premises. There are no bodies of water or weapons/firearms or ammunition in the home or on the property. The fire extinguisher size (2A10BC) meets requirements and is fully charged, located by the entrance door. The smoke detector and carbon monoxide detector in one located on the hallway by the family room is operational. Applicant’s Pediatric CPR/FA certification is valid through May 6, 2025. Mandated Reporter training certificate expires on April 18, 2025. Licensee stated that she rents the home. The Fire Safety Inspection Request (STD 850) was approved by the local fire marshal and GRANTED the capacity increase to twelve (12) children on October 3, 2024. Licensee does not have a Landlord Consent Form.
Licensee will be using the following areas for childcare: family room, bathroom 1 by the hallway, bedroom 1, dining room and backyard. Off limits areas of the home are the living room, kitchen, garage, and all the bedroom upstairs which is barricaded by a safety metal gate. Off limit areas has been inaccessible with the use of doorknob covers.

The home is equipped with age-appropriate toys and are in good condition. Applicant was reminded of requirements for children’s records, child abuse reporting, unusual incident reporting, immunizations, criminal background clearance procedures and policies, and posting requirements.
The last fire drill was done on July 2, 2024. Children and staff records were reviewed.
