25 | On November 8, 2023 at 9:00 AM, Licensing Program Analyst (LPA) Sherlynn Banas conducted an announced Pre-Licensing inspection for the applicant. Upon arrival, LPA met with Shoaib Fazly (applicant’s brother) who guided the LPA with directions to the FCCH. Upon arrival, Razia Fazly greeted LPA Banas. The one story home was toured and inspected to ensure an environment safe for the care and supervision of children. The fire extinguisher (3-A:40-B:C), carbon monoxide detector, and smoke detector meet requirements and are operational which are located in the living room. All hazardous items were latched/locked and secured out of reach of children. There is a working phone at the facility. There are no bodies of water in the home. Applicant states that there are no weapons in the home. Applicant states that they have sufficient financial resources to sustain the license. CPR and First Aid expire on October 24, 2024. Preventative health practices course was completed on October 26, 2022 which includes lead poison prevention training. Mandated Reporter Training was taken last November 2, 2022. Staff immunization requirements were met. Required documents have been posted. Applicant rents the home. The applicant has toys.
Applicant will be using the following rooms for childcare: Living room, kitchen, bathroom, and bedroom 1 are used as accessible for day care. The front yard will be accessible for outdoor play area. Off-limits areas include bedroom # 2, outside living room are inaccessible. LPA asked if there is a separate room or place for isolation when one child/ children were sick, and stated that bedroom 1 will be used. There is no garage. Property owner/Landlord notification is on file. Landlord consent on file.
Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. |
