25 | On 10/13/2023 at 9:45am, Licensing Program Analysts (LPAs), Selina Siao and Martha Avila conducted a pre licensing inspection with applicant. The 4 bedrooms, 2 bath one-story house was toured and inspected to ensure an environment safe for the care and supervision of children. The home has a fully charged fire extinguisher size 2A10BC, smoke and carbon monoxide detector that meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children during the inspection. The home does not have any bodies of water. Applicant stated that the home doesn't have any weapon. Applicant has an EMSA approved pediatric CPR and First Aid card that are current due to expire in October 2025. Preventative health practices course was completed on August 12, 2023, which includes lead poison prevention training. Mandated reporter training expires September 26, 2025. A review of the application and records on this date indicates that applicant, her husband and her son are the current adult residents at the home with the require TB clearances, caregiver background checks and child abuse clearances. Applicant has the required measles and TDAP immunization.
Because the applicant, rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord.
The applicant has not obtained a signed Property Owner/Landlord Consent form (LIC9149). Without this consent, the applicant understands that, once licensed, they can operate with a maximum capacity of 12 children. If property owner/landlord consent is obtained in the future, the applicant is advised that a new Application for a Family Child Care Home License (LIC 279) must be submitted with a change of capacity fee of $25, to increase the capacity and provide care to 14 children. |
