25 | On 4/5/24, Licensing Program Analyst (LPA) Keturah Lane conducted a second pre-licensing inspection with the applicant to approve corrections from the 1st pre-licensing visit on 3/15/24. Upon arrival, LPA was greeted by applicant Mariews Tibo and also present was applicant's mother Vivian Elias and Muwafaq Toma.
During this visit, LPA observed the following corrections:
1. Screw locks were installed on both sliding glass doors making backyard off-limits and inaccessible to children.
2. Latches were installed on sink cabinet and drawers in kitchen
3. Latches were installed in bathroom #2 (upstairs) & shampoos were removed from around tub (up high out of reach)
4. Fireplace had a screen screwed onto the fireplace to make it inaccessible
5. Garage had new latch installed with a lock making it inaccessible
6. Upstairs bedrooms (#1 & #2) had screw locks installed on windows
7. Upstairs bedrooms (#3 & #4 - off-limits) had latches installed with locks to keep them inaccessible.
7. Retractable mesh safety gates were installed at bottom and top of stairs making the stairs inaccessible. LPA checked gates for sturdiness.
8. Applicant had the correct size fire extinguisher that was operational.
9. Applicant had a functioning carbon monoxide detector.
10. Applicant had completed sketch of both floors and LPA obtained photo for the file
11. Applicant completed mandated reporter training with certificate date of 4/4/24 and expires 4/4/26.
Applicant did not have toys and equipment available. Applicant stated he would obtain some toys and send a picture to LPA Lane later today. Once corrections have been received, a license for 8 children may be granted upon final file review. Applicant agreed to comply with all regulations and laws governing family child care homes. Exit interview conducted and report was reviewed with applicant Mariews Tibo. |
