On November 16, 2023 at 8:45AM, Licensing Program Analyst(LPA) Nancy Diaz conducted an announced Pre-Licensing inspection for a change of location with the applicant, Farkhonda Bibi Danishyar. Her husband, Elyas Ahmad was also present during the prelicensing to help translate in Farsi.

This 3-bedroom, 2-bathroom single family home was toured and inspected to ensure an environment safe for the care and supervision of children.

The applicant provided proof of control of property (lease agreement).
Because the applicant rents the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).
Applicant maintains a carbon monoxide detector and a regulation-size fire extinguisher in the home that meets regulation requirements. The carbon monoxide detector was tested today and deemed to be operable. All hazardous items were latched and secured under the kitchen and bathroom sink and are out of reach of children. Mrs. Danishyar stated that she does not maintain any weapons. Applicant maintains a first aid kit in the home.

A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Control of property was provided to the department via copy of lease/rental agreement. First Aid and CPR will expire on May 20, 2025. Mrs. Danishyar is registered to complete Preventative Health Practices course on November 28-29, 2023. Mandated Reporter Training was completed on November 1, 2023.

The daycare children will have access to: living room, family room, dining, kitchen, hallway bathroom and back fenced yard. Off limit areas are: All 3 bedrooms and garage.

LPA reviewed the children's files that the licensee is currently maintaining for 4 children. File review revealed that licensee did not maintain form LIC 995A. LPA also reviewed the required posting on the wall, she must post the following: LIC 610A, LIC 999A, Car seat law and the license once issued.

Applicant has met all immunization requirements per SB792 and have completed the AB1207 Mandated Reported Training. Applicant is reminded to make anything that reads, "Keep Out of Reach of Children" inaccessible to children.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Applicant states that she will comply with all regulations and laws governing family child care homes and that she is financially secure to operate a family child care home for children. LPA reviewed this report with Applicant prior to obtaining her signature.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

MyChildCarePlan.org
Applicant was informed of the myChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Megan’s Law
On this date, 11/03/2023 the California Attorney General – Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California’s Megan’s Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Capacity shall include children under age 10 who reside at the licensee’s home.

CORRECTIONS NEEDED PRIOR TO LICENSURE:
1. Install a barricade to make fire place inaccessible to children in the living room.
2. Install a smoke detector that is operable.
3. Install door knob cover to the master bedroom door to make it inaccessible to children.
4. Obtain livescan fingerprint clearance for father - Nasir Ahmad Danishar who is visiting and is staying for an undetermined amount of time.
5. Cover power outlets in the living room.
6. Obtain signed LIC 995A for all children in care.
7. Post required forms on the wall.

Corrections shall be completed no later than November 30, 2023. Mr. Danishar shall contact this LPA to schedule a follow-up inspection.

An exit interview was conducted with the applicant. A copy of this report and appeal rights were provided.
