25 | On 1/19/2024 @ 8:30AM, Licensing Program Analyst (LPA) Nancy Diaz conducted an announced follow-up prelicensing inspection. LPA met with applicant Farkhonda Bibi Danishyar and her husband, Elyas Ahmad Danishyar was also present to help translate in Farsi. The purpose of this inspection is to observe corrections as requested on 11/16/2023.
The following corrections were observed today:
1. A barricade was observed installed to make fire place inaccessible to day care children.
2. Smoke detector was tested and deemed operable.
3. Door knob cover was installed to the master bedroom door to make it inaccessible to day care children.
4. Livescan fingerprint clearance was obtained for Nasir Ahmad Danishyar.
5. Power outlets in the living room were now covered.
6. LIC 995 A were on file for day care children.
7. Required forms were observed posted.
Mrs. Danishyar has not obtained a signed Property Owner/Landlord Consent form (LIC9149). Without this consent, the applicant understands that, once licensed, they can operate with a maximum capacity of 6 children. If property owner/landlord consent is obtained in the future, the applicant is advised that a new Application for a Family Child Care Home License (LIC 279) must be submitted with a change of capacity fee of $25, to increase the capacity and provide care to 8 children.
Exit interview was conducted with Mrs. Danishyar. Report was reviewed and provided today.
License is granted for maximum capacity of 6 children with no more than 3 infants or 4 infants only.
Capacity shall include children under age 10 who reside at the licensee’s home. |
