On 1/22/2026 at 10:30 AM Licensing Program Analyst (LPA), Mahjoba Mohsini made an unannounced visit to investigate the complaint received on 1/20/2026 regarding the above referenced allegation. LPA was greeted at the door by the Licensee, Fenik Khoshnaw and was granted entry after identifying self and disclosing the purpose of the visit. LPA toured the facility and observed 4 children (ages 20 months to 5 years old). Present at the home was also the facility helper, Licensee's husband (Rawand Mamsini). The facility is within ratio and capacity.
Information from a third party indicated the licensee was over capacity on 1/15/26, with seven children in care and no proof that the seventh child was enrolled in school. During the investigation, the LPA conducted observations, interviewed the licensee, and reviewed relevant documents. The evidence confirmed the seventh child was not enrolled in school on that date, placing the licensee out of ratio. The information met the preponderance of evidence standard; therefore, the allegation is substantiated. A Type B deficiency is cited on the accompanying LIC 9099D.
Exit interview was conducted and report was reviewed with the Licensee Fenik Khoshnaw. Appeals Rights were verbally reviewed and provide along with Notice of Site visit. Notice of Site Visit must remain posted for 30 days.
