25 | On October 1, 2024, at 2:10 PM., Licensing Program Analysts (LPAs) Sherlynn Banas and Adriana Macias conducted an unannounced case management inspection in reference to the licensee’s request to make a change to an off-limits area in the daycare floor plan at the second floor . Upon arrival LPAs met with Licensee, Mariam Nooristani and proceeded to tour the facility inside and outside per facility sketch. There were 3 children in care and 1 school age of her own. Also present was the licensee’s husband, Omar Alemi, who left at 2:40 PM. The facility was observed operating within ratio and capacity.
The carbon monoxide and smoke alarm in one and fire extinguisher were all operational. The garage, second floor bedrooms, and bathroom were off-limits except the masters bedroom . The second floor was barricaded by a plastic safety gate. The licensee states that masters bedroom at the second floor was requested to be accessible. All poisons, cleaners and hazardous items in the room are inaccessible to children through latches, locks, and/or placed up on high surfaces. An updated facility sketches reflecting masters bedroom as an accessible room has been submitted and accessible for children in care.
The CPR/FA for Mariam Nooristani expires on March 9, 2026. Mandated Reporter expires on 3/14/2026. The hours of operation are from 7:00 AM. to 5:30 PM.
No deficiencies are cited.
Exit interview was conducted. LPAs provided Notice of Site Visit and observed being posted. |
