On 6/25/2024 at 12:30 PM, Licensing Program Analyst (LPA) Victoria Hernandez conducted an announced Pre-Licensing inspection with the applicant Jean Isnadin Louis and interpreter Jean Laurent Jean Baptiste. Applicant primary language is Haitian Creole. LPA was granted entry after showing badge, identifying self, and disclosing nature of visit. The 2 bedroom, 1 bathroom, downstairs apartment home was toured and inspected to ensure an environment safe for the care and supervision of children

Applicant will be using the Living Room, Bathroom, and Kitchen for childcare. Off limit areas are Bedroom 1 and Bedroom 2 and are inaccessible by use of doorknob covers. The applicant has sufficient toys and equipment available. The applicant will utilize a nearby park for outdoor activities. The applicant understands there must always be direct supervision while at the park with children.

The fire extinguisher located in living room, carbon monoxide detector, and smoke detector meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. Applicant states that there are no weapons in the home. Applicant states that they have sufficient financial resources to sustain the license.

First Aid and CPR expire on 4/2026 and preventative health practices course was completed on 4/17/2024. Applicant completed Mandated Reporter Training on 4/28/2024. Staff immunization requirements per SB792 were met.

A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
