25 | On September 20, 2024, at 9:01 AM., Licensing Program Analyst (LPA), Sherlynn Banas met with Licensee Judith Chery, for the purpose of increase in capacity from eight (8) to fourteen (14) children. Licensee’s minor daughter who was sleeping at the time of inspection was also present. Watson Antoine (husband) arrived at the home at 10:15 AM. There were no daycare children when LPA Banas arrived at facility. Licensee has 5 enrolled children.
LPA Banas toured the home. There are three bedrooms and two bathrooms. All required forms were posted. There is a working telephone on the premises. There are no bodies of water or weapons/firearms or ammunition in the home or on the property. The fire extinguisher size (2A10BC) meets requirements and is fully charged, located by the dining area. The smoke detector and carbon monoxide detector in one located on the hallway by the bedrooms is operational. Applicant’s Pediatric CPR/FA certification is valid through July 2025. Licensee stated that she rents the home. The Fire Safety Inspection Request (STD 850) was approved by the local fire marshal and GRANTED the capacity increase to fourteen (14) children on September 16, 2024.
Applicant will be using the following areas for childcare: living room, bathroom 1 by the hallway, bedroom 2, and bedroom 3. Off limits areas of the home is the dining room, kitchen which are made inaccessible by a metal safety gate, master bedroom, bathroom 2, and backyard. Off limit areas has been inaccessible with the use of doorknob covers. Licensee stated that she brings the children to Madison Park for outdoor activities.
The home is equipped with age-appropriate toys and are in good condition. Applicant was reminded of requirements for children’s records, child abuse reporting, unusual incident reporting, immunizations, criminal background clearance procedures and policies, and posting requirements. |
