On 12/23/24 at 8:05 AM, Licensing Program Analysts (LPA) Adriana Macias conducted an unannounced case management inspection in reference to the licensee’s request to an increase of capacity. LPA Macias met with Licensee Zabihullah Karim and also present were other adults that live in the home Barath Bibi and Wahidullah Razz. Physical therapist for one of the children showed up later. There were also 4 children present who live in the home. This visit is to verify that the licensee remains in substantial compliance with the health & safety standards as required by regulations governing large family childcare homes. Licensee has all appropriate forms posted. LPA confirmed with licensee that all adults residing/working in the home have criminal record/TB clearances. Facility Records and Children’s Records were reviewed. LPA suggested that all children’s records be printed and kept in one place, such as a folder or binder. The home appears to be large enough to comfortably accommodate 14 children.

Fire clearance was granted and received from the Heartland Fire & Rescue Department on 12/19/24.

Licensee has provided adequate space for the children to eat, sleep and play within the home. Areas used for child care are the first floor which includes the Living Room, Kitchen, Bathroom 3, and Backyard. Off limit areas are the entire second floor, and garage and are inaccessible by use of doorknob covers and a gate. Backyard is fully fenced and accessible for outdoor activities. The fire extinguisher, smoke detector, and carbon monoxide detector meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. There are no bodies of water in the home and licensee states that there are no weapons in the home.

Licensee’s First Aid and CPR certifications expire on 05/06/25 and helper’s First Aid and CPR certifications expire on 09/26/26. Licensee and helper have required immunizations. Licensee is exempt from Mandated Reporter Training as their primary language is Pashto. The helper is exempt from Mandared Reporter Training as their primary language is also Pashto.

Provider is hereby reminded of the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms; corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer.

No corrections are needed. Licensee did not obtain a Landlord Consent Form (LIC 9149), therefore, a license for 12 children may be granted upon final file review. Applicant agreed to comply with all regulations and laws governing family child-care homes.

Exit interview conducted and report was reviewed with the licensee Zabihullah, Karim. Appeal Right were also printed and issued to licensee.
