25 | On 8/25/2025 at 9:45 am Licensing Program Analyst (LPA) Gerald Poindexter conducted an unannounced annual inspection. Upon arrival, LPA met with licensee Parthvi Jariwala. Present in the home were 6 daycare children, including 3 children under 24 months. The licensee’s own minor child was also present. Licensee states operating are hours from 8 am to 5:00 pm. The licensee was provided with the Inspection Checklist (LIC 126). The 2-bedroom, 1-bath, ground floor home was toured and inspected to ensure an environment safe for the care and supervision of children.
The licensee has provided adequate space for the children to eat, sleep and play within the home. First floor living room, first floor half bathroom, first floor dining room, and first floor nook. Off-limit areas include: First floor master bedroom, first floor master bathroom, first floor storage, first floor kitchen, second floor attic, second floor bedroom 1, second floor bedroom 2, and second floor bathroom. Off-limit areas are inaccessible to children through the use of gates, child locks and gate at bottom of the stairs. The garage is also off limits and is kept inaccessible through the use of a door knob cover on the door. Applicant has a partially fenced backyard. The side yard is open to the front yard, however, applicant placed a small movable baby gate to prevent children from going to the front yard. LPA advised that continuous visual supervision is always required during outdoor activities. The facility has sufficient toys and equipment available. No body of water was observed during the time of inspection.
There is a fully charged fire extinguisher, smoke and carbon monoxide detector that meet requirements and are operational. Poisons, cleaning compounds, medications and other hazardous items are inaccessible to children via latches/locks. Adequate heating and ventilation are provided. There is a working telephone and email address. Licensee stated there are NO firearms and weapons in the home. CONTINUED ON PAGE 2
