On 12/3/2024 at 9:00 A.M, Licensing Program Analyst (LPA) Edleen Montesa conducted an announced Pre-licensing inspection with the applicant, Kunar Safi. LPA was granted entry after showing badge, identifying self, and stating the reason for the visit. The five bedroom, three bathroom, two story home was toured and inspected to ensure an environment safe for the care and supervision of children.

Applicant will be using the living room, kitchen, Bedroom 1 beside living room, for day care operations. Off limit areas are the garage, backyard, and all of the bedrooms and bathrooms located on the second floor (Bedroom 2, Bedroom 3, Bedroom 4, Bedroom 5, Bathroom 2, and Bathroom 3) are inaccessible by use of door knob covers, child safety gates, locks, and safety latches. The applicant has play materials, and equipment available in the home. LPA inspected the backyard and noticed that the area is not safe for childcare due to the hazards currently present such as broken glass on the ground, leaning wooden side fence, and unlocked side gate. There is a door that leads to the backyard and entry to the area has been made inaccessible through the use of a safety door knob cover and a lock. Applicant will instead utilize nearby park for children’s outdoor play and has been reminded that full visual and active supervision of daycare children is required at all times.

The fire extinguisher is located in the kitchen, carbon monoxide detector is located in the kitchen, and smoke detector is located in the living room, meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. There are no bodies of water on the property. There is a fireplace located in the living room and has been screened and barricaded with a child safety gate. There is an air vent present at the bottom of the stairs. Per applicant, the air vent is where the air filter is located. Applicant showed LPA the air filter and the air filter was present. LPA confirmed with applicant that the heating vent is centralized and the air vents used for heating are located at the ceiling. The stairs that lead to the second floor is barricaded with a child safety gate at the bottom of the stairs and at the top of the stairs. Applicant states that there are no weapons in the home. Applicant states that they have sufficient financial resources to sustain the license. (See pg.2 continuation on LIC-809C...)

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The applicant provided proof of control of property. Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).


CPR and First Aid certification expires on 05/15/2026 and Preventative Health Practices course which includes nutrition and lead training, was completed on 05/27/2024. Applicant completed Mandated Reporter Training on 08/02/2024. Immunization requirements per SB792 were met. Required documents have been posted. LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.

The New Provider packet was reviewed with the applicant including information on ratios and capacity, child abuse reporting, unusual incident reporting, children’s records, immunizations, personnel records, car seat law, Shaken Baby Syndrome, Sudden Infant Death Syndrome, effects of lead poisoning, required facility postings, and community resources. Applicant was reminded that corporal punishment, smoking, walkers, exersaucers, jumpers, and bouncy seats are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. On this date, 12/03/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

LPA discussed and provided applicant with the following information:

· For common questions or questions regarding licensing requirements to contact the Child Care Licensing duty line at 619-767-2248.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice) or (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-carelicensing/subscribe and select the Child Care option to receive email communication.

No corrections are needed. A license for 8 children may be issued upon final file review.

Applicant agreed to comply with all regulations and laws governing family child-care homes. Exit interview conducted and report was reviewed with the applicant, Kunar Safi. A notice of site visit was given to applicant and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
