On 12/27/24 @ 8:30 a.m., LPA Mahjoba Mohsini conducted an announced comprehensive visit for the purpose of a pre-licensing inspection. LPA was greeted at the door by the Applicant, Katya Talavera and also present was her husband, Brandon Quezada.  LPA was granted entry after identifying self and disclosing the purpose of the visit.

Applicant Katya presented evidence of lease agreement.  LPA confirmed that all adults living in the home have been fingerprint cleared and associated with the facility and have met all the immunization requirements.  Applicant has provided evidence of the completion of the required 16-hr. Health and Safety training to include a Pediatric First Aid/CPR certificate with ABC, valid through 07/01/2026 and Mandated Reporter Training, valid thru 11/15/2026. The applicant, has not yet obtained a signed Property Owner/Landlord Consent form (LIC9149). The applicant understands that, once licensed, they can operate with a maximum capacity of 6 children unless they provide Landlord Consent form prior to license bing issued.

A tour of the two story home, 4 bedrooms, 2 and 1/2 bathrooms, backyard was conducted. The removable gate will be moved to to provide access to the half bathroom located downstairs while making the stairs inaccessible. Backyard has steps, some rocks the applicant plans to make safer by adding additional removable fences. The play structures would need to become immovable or anchored. The garage door has a lock but Applicant will ensure additional safety by adding child safety lock.
Sharps, toxins and cleaning supplies have been inaccessible through use of locks, latches and keeping away from reach. LPA tested the smoke alarm and carbon monoxide detectors, located by the stairs and the hall and they were operational.  The fire extinguisher is fully charged, easily accessible in the kitchen and meets the size requirements (2A10BC). Applicant stated there are no weapons in the home, the neighbor has a pool but Applicant's back yard has fence to block access. The home has central heating and AC. 
The home is kept clean and orderly with heating and ventilation for safety and comfort.

Outdoor play will take place in the fenced back yard. There is a sufficient amount of age-appropriate toys, books and games for children, in good condition. LPA reviewed LIC 311D with the Applicant.

LPA reminded applicant about safe sleep regulations and the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed Applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date, 12/26/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Katya Talavera was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed.

Applicant, was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
