25 | On 12/10/24 at 8:00AM, Licensing Program Analyst (LPA), Annette Sutherland, met with Licensee, Fatima Hamidi, for the purpose of an announced pre-licensing inspection for a change of location. Licensee was previously licensed at facility 376100786. Licensee understands that effective today, the old facility will be closed, and care can no longer take place at that location. Licensee's spouse Samiiullah Hamidi was present during today's inspection. Facility operating hours will be from 7:30 AM – 10:00 PM Monday thru Friday.
The home is a one story, 2 bedrooms 1 bathroom which includes kitchen, a living room, and backyard. Off-limits areas include Bedroom #2 and have been mad inaccessible with the use of doorknob cover. Licensee understands children may not enter off-limit areas.
A health and safety inspection were conducted on the interior and exterior of the home. Toxic and hazardous items, sharp knives, and medications are inaccessible to children. LPA observed a functioning smoke detector, carbon monoxide detector, and a fully charged 2A10BC fire extinguisher within the home. LPA advised the licensee that if there are ever any poisons at the home, all poisons must be inaccessible to children. Licensee stated there are no weapons or bodies of water on the premises.
Licensee has completed the required Preventative Health and Safety course with the Lead Poisoning Prevention training. Current EMSA approved pediatric CPR/First Aid trainings were completed and expires12/2025. Licensee is exempt from Mandated Reporter Training due to language barrier Pashto. Licensee understands CPR training must be completed every two years.
LPA discussed capacity and supervision with applicant. Licensee understands that if an unusual incident occurs, licensing is to be notified via phone call, e-mail or fax within 24 hours and the Unusual Incident Report LIC 624 shall be submitted within 7 days to remain in compliance. LPA discussed Type A/B citations, open door policy, fire drills, children’s personal rights with the Licensee. Licensee understands a current roster of children enrolled must be available and maintained for a period of three years, even after children are no longer attending.
