On October 8, 2025 @ 1:00PM, Licensing Program Manager (LPM) Joelle Redding, License Program Analyst (LPA) Nancy Diaz conducted an Office Meeting with applicant Umia Noori. Her husband, Noorulhaq Noori was also present and helped translate in Pashto. The purpose of this meeting is to discuss Mrs. Noori's recent submission of an application for a Family Child Care License.

Mrs. Noori submitted form LIC9149 (Landlord Consent) form on 9/11/2025 as part of the application packet. The form obtained from applicant displayed the handwritten name of the landlord, the name of the applicant, the address and what was said to be the landlord's signature. On 9/29/2025 LPA contacted the landlord, Victoria Ramirez (via email) and confirmed that the signature on form LIC9149 was neither her name or her handwriting. LPA spoke with applicant Umia Noori on 9/29/2025, she stated that she does not know who signed off on LIC9149. On 9/30/2025, Applicant submitted updated LIC9149 signed by Raul Bracamontes.

During today's meeting, applicant Umia Noori admitted to forging the signature on form LIC9149. Applicant apologized and said that she was sorry. She stated that it was a mistake. Applicant regrets that she forged the signature on the form. Applicant promised that this will never happen again.

During today's meeting the Department discussed and provide Laws and Regulations:

Staffing Ratio and Capacity 102416.5: (a) The capacity specified on the license shall be the maximum number of children for whom care may be provided at any one time. (b) For a Small Family Child Care Home, the maximum number of children for whom care may be provided at any one time, including children under age 10 who reside at the licensee's home, shall be one of the following: (1)Four infants; or (2) Six children, no more than three of whom may be infants; or (3) More than six and up to eight children, without an additional adult attendant, only if the criteria in Section 1597.44 of the Health and Safety Code are met.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Health and Safety Code section 1597.44 states:
A small family day care home may provide care for more than six and up to eight children, without an additional adult attendant, if all of the following conditions are met:
(a) At least one child is enrolled in and attending kindergarten or elementary school and a second child is at least six years of age.
(b) No more than two infants are cared for during any time when more than six children are cared for.
(c) The licensee notifies each parent that the facility is caring for two additional schoolage children and that there may be up to seven or eight children in the home at one time.(d) The licensee obtains the written consent of the property owner when the family day care home is operated on property that is leased or rented.

Health & Safety Code 1596.885: The department may deny an application for or suspend or revoke any license, registration, or special permit issued under this act upon any of the following grounds and in the manner provided in this act: (a) Violation by the licensee, registrant, or holder of a special permit of this act or of the rules and regulations promulgated under this act. (b) Aiding, abetting, or permitting the violating of this act or of the rules and regulations promulgated under this act. (c) Conduct which is inimical to the health, morals, welfare, or safety of either an individual in or receiving services from the facility or the people of this state. (d) The conviction of a licensee, or other person specified in Section 1596.871, at any time before or during licensure, of a crime as defined in Section 1596.871. (e) Engaging in acts of financial malfeasance concerning the operation of a facility, including, but not limited to, improper use or embezzlement of client moneys and property or fraudulent appropriation for personal gain of facility moneys and property, or willful or negligent failure to provide services for the care of clients.

In addition to the above, the Department also discussed and provided the following Laws and Regulations: 102417 Operation of a Family Child Care Home, 102421 Child’s Records, and 102391 Inspection Authority. The Department reviewed and provided the capacity handout with Mrs. Noori, and she stated her understanding.

Mrs. Noori was also provided with the CDSS Child Care Licensing (CCL) Family Child Care Providers Resource link with instructional videos: https://ccld.childcarevideos.org/family-child-care-providers/.

It is recommended for Applicants to review the video(s) including but not limited to: “Record Keeping in Family Child Care”, “Child Care Reporting Requirements”, “Community Care Licensing Inspection Authority”, and “How Many Children Can Attend a Family Child Care Home”.

Mrs. Noori was also provided with the Safe Sleep website: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep and Child Care Law Center website: www.ChildCareLaw.Org. Applicant was advised to regularly visit the Community Care Licensing Website: www.ccld.ca.gov for quarterly updates and regulations. Licensee stated she has signed up to receive new Provider Information Notices (PINs). The Regional Office advised that the Duty Line is available Monday – Friday 8:00AM to 5:00PM at (619) 767-2248.

Applicant, Umia Noori, states she understands she must abide by the Health and Safety Code and Title 22 Regulations in the operation of her Family Child Care Home.

A copy of this report, and all referenced resources and documents, were reviewed and provided to the Applicant, Umia Noori, at the conclusion of this meeting.
