25 | On 4/6/26 at 1:30, Licensing Program Analyst (LPA) Gerald Poindexter, while conducting a visit for another matter, was informed of the presence of an uncleared adult residing in the licensee’s home. Based on record review and interview conducted with the licensee, Abdalla Farah, his wife, Farhio Yusuf lives part-time in Phoenix, AZ, but has been residing with the licensee on weekends since November 2025 -- during operating hours, and has assisted and interacted with children in care. Licensee states operating hours are from 3:30 to 9:30 pm, 7 days per week. The licensee has only recently, on 3/30/26, submitted required documentation to the Department to have Ms. Yusuf designated as a helper/assistant. Ms. Yusuf has not submitted LIC9163 Live Scan documentation and has not obtained the required DOJ, FBI and Child Abuse clearance for more than five days. A civil penalty of $500 will be assessed today. See LIC 809-D for deficiency cited.
LPA Poindexter informed Licensee Abdalla Farah that this report dated 4/6/26 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Poindexter informed the licensee Abdalla Farah to provide a copy of this licensing report dated 4/6/26 that documents any Type A citation to the parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
LPA provided Licensee with Live scan locations, community resources, LIC 9163 - Request for Live Scan Service application, and LIC9224. Exit interview was conducted with the licensee, Adballa Farah. Appeal Rights and licensing report was reviewed with the Licensee. Signature at the bottom of this report confirms receipt. A Notice of Site Visit was posted and must remain posted for 30 days.
