On 6/23/23 at 9:45 AM, Licensing Program Analyst (LPA) Keturah Lane visited the facility to conduct an annual inspection. Upon arrival LPA met with staff member Alma Vallejan and proceeded to tour the facility. Staff member Alma Vallejan stated that Director Andrea Dagostino was not at the facility today and she would assist LPA with inspection. During today's inspection, there were 49 children with 5 staff in 5 classrooms. Appropriate ratios and capacity were observed. Appropriate care & visual supervision were also observed during the inspection while children were playing outside and engaged in classroom activities.

At 10:00 AM, LPA observed a child’s bench in room #4 that has foam accessible through a hole that needs repair. LPA observed several chairs, tables, toys in the classrooms that were visibly dirty. LPA observed play kitchen in room #3 to have dirty fingerprints and LPA was able to easily wipe them off (in a small section) with a wipe. LPA observed the carpet in room #3 needs cleaning and had several gray/brown spots. LPA observed several napping mats have rips/tears in them and the foam is accessible. LPA inspected the first-aid kit on site and it is missing scissors, adhesive tape and tweezers. At 10:15 AM, LPA observed Clorox wipes accessible to children in three classrooms (on desks and in unlatched cabinets on lower shelf) and a bottle of ibuprofen accessible in an unlatched drawer in Room #7. Rooms have adequate heating, lighting, ventilation. Floors appear to be clean and safe. Drinking water is readily accessible. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a kitchen which is clean and free of hazards. Menu is posted. Adequate food is available for snacks. Children bring their own lunches for lunchtime. Cleaning supplies are kept separate from food and are inaccessible to children (except for Clorox wipes). Storage containers for solid waste have tight-fitting covers and are kept in good repair. Director stated there are no firearms or other weapons on the premises. All foods/beverages capable of rapid spoiling are stored in covered containers at 45 F or less. The facility appears to be free of insects and rodents. There is an operational carbon monoxide detector at the facility. Last fire drill was conducted on 4/28/23 and was documented.

Outdoor play area is a fenced playgrounds with sufficient material for cushioning. LPA observed that the equipment needs cleaning. There are no bodies of water at this facility. Climbing structures, swings and slides are securely fixed to the ground. Area has canopies used for shade. Equipment is age appropriate. Area has drinking water readily accessible, and grounds are free of debris or potential hazards. In the upper playground, LPA observed a ship/boat was damaged on one side – the plastic was broken and needs repair.

LPA reviewed medication storage and observed two epi-pens without prescription labels. Personnel records contain health screening documentation and at least one staff member has current CPR and First Aid certifications. All staff have completed mandated reporter training and required immunizations. Each personnel record contains documentation of educational background and training. Sign ins were reviewed. Children’s records contain admission agreements and medical assessment.

A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
