On 8/22/25 at 11:10 a.m. Licensing Program Analyst (LPA) Renita Rodriguez made an unannounced complaint visit for the complaint received on 4/23/25 for the purpose of delivering findings on the above reference allegation. LPA was granted entry after identifying self, showing badge, and disclosing the reason for the visit. Ratios observed 17 infants and 5 staff.

Based on the information obtained during interviews and observations LPA observed outdoor infant toys and they were found to be dirty and unsanitary. LPA observed visible grime and felt dirt when touching the toys. Director states on 8/16/25 and 8/17/25 the parking lot next to the infant outdoor play area was repaved. Director states this caused the toys to get dirty. Director states the toys were wiped down on 8/18/25. Director states a deep cleaning of the facility and outdoor equipment is scheduled for 8/29/25.

The allegation is valid because the preponderance of the evidence has been met, therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12, Chapter 1) the deficiency is being cited on the attached LIC 9099D. The Notice of Site Visit was provided, and LPA observed posting. Licensee is advised it must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Soshia Garcia. A notice of site visit was given and must remain posted for 30 days.
