On 2/14/22, Licensing Program Analyst (LPA), Tyra Block, conducted an unannounced case management visit to discuss terms of the stipulation and answer any questions licensee might have. Present at the facility were 18 children and 2 staff in room B-5. Barbara arrived later during the visit. LPA discussed the probationary period and providing copies of the stipulation/ accusation to parents and obtaining signed acknowledgement of receipt and/ or confirmation to be maintained in each child's file. LPA provided copies of the new probationary license and obtained a copy of the facility roster.

Licensee will submit proof of receipt by parent of notification of stipulation by Friday, 2/18/22.

A Notice of Site Visit was provided and must be posted for 30 days.
