On May 3, 2023 at 1:30 p.m., Licensing Program Analyst (LPA) Leilani Curtis conducted an unannounced Plan of Correction Inspection to verify that the citations issued on LPA Curtis’s Annual Inspection dated 2/15/23 have been addressed. LPA met with Director Rebecca Jaime and toured the facility. Also present were 19 napping children with 2 staff members. Appropriate ratio/capacity was observed. Staff members have the required background clearances and are associated to the facility.

LPA reviewed facility and staff records. All citations issued on 2/15/23 have been corrected and cleared.

No deficiencies cited during today’s inspection.

An exit interview was conducted with the director and appeal rights (LIC 9058 1/16) were discussed. The director’s signature on this form acknowledges receipt of these rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed the director post notice of site visit.
