On 6/15/2023 at 2:35 p.m. Licensing Program Analyst, Leilani Curtis, conducted an unannounced case management inspection. Upon arrival LPA met with Director Rebecca Jaime, advised her of the purpose of the visit and proceeded to tour the facility. There were 25 children with 3 staff members present. Appropriate ratio/capacity was observed. Staff members have the required background clearances and are associated to the facility.

LPA previously reviewed video dated 4/24/23 of the preschool classrooms. On the video of the Mighty Adventurers classroom staff #1 (S1) can be seen leaving the classroom and entering the restroom several times while children were laying down on their cots during nap time. The restroom is a separate room located within the classroom. The staff member was unable to visually observe & supervise the children on their cots while she was in the restroom. On this same video a child (C1) left the classroom and went into the restroom unsupervised/unattended. LPA discussed providing proper care and supervision to the children in care with Director Jaime.

No deficiencies are cited.

An exit interview was conducted with Director Jaime and Appeal Rights (LIC 9058) were discussed. The director’s signature on this form acknowledges receipt of these rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed the director post notice of site visit.
