On 6/27/24, Licensing Program Analyst (LPA) Saraliz Velando conducted an unannounced visit for the purpose of delivering findings for complaint received on 4/5/24. The LPA met with Owner, Rafat (Rocky) Ilaian and toured the facility. There were 8 infant children present and 3 staff (including one fully qualified teacher). Based on the information obtained from observation, file reviews, parent, and staff interviews, it was determined that staff commingled day care children. Interviews revealed sufficient information to prove that children of infant age and children of preschool age had been dropped off and placed together in the same room while waiting for additional staff to arrive. It was also determined that staff did not provide a proper temperature for infants in care. On 4/15/24, LPA observed the thermostat at 65 degrees in the Infant 1 room. The preponderance of the evidence has been met and therefore, the above allegations are found to be SUBSTANTIATED. Refer to the next page LIC 809-D for deficiencies. The exit interview was conducted with Owner, Rafat (Rocky) Ilaian. Appeal Rights and a copy of the licensing report was provided. A notice of site visit was posted and must remain for 30 days.

The licensee states that he will have a staff meeting to discuss keeping infant children (ages 0-24 months) separate from the preschool children (ages 2 thru Kindergarten) and submit proof of agenda and attendance to the dept by 7/5/24.

Based on interviews conducted by LPA, infants under 1 year old and preschool age had been allowed to use Infant Room 2 during drop off times. This poses a potential health, safety or personal rights risk to the children in care.
