On May 3, 2023 at 12:50 p.m., Licensing Program Analyst (LPA) Leilani Curtis conducted an unannounced Plan of Correction Inspection to verify that the citations issued on LPA Curtis’s Annual Inspection dated 2/15/23 have been addressed. LPA met with Director Rebecca Jaime and toured the facility. Also present were 17 napping children with 4 staff members. Appropriate ratio/capacity was observed. Staff members have the required background clearances and are associated to the facility.

LPA reviewed facility and staff records. On LPA Curtis’s annual inspection report dated 2/15/23, LPA observed five infant bottles in room #3 labeled with the child’s name but without a date. During today's inspection, LPA observed that a child (C1) has food at the facility labeled with his name but without a date in room #3. The director states that all food and bottles are returned to the child's parent/guardian at the end of the day. During the 2/15/23 inspection the director stated that she would conduct a staff meeting regarding infant food care service and send LPA a copy of the meeting agenda and staff sign in sheet. The director states that the staff meeting was not conducted. The director states that she will conduct the meeting and send the agenda & staff sign in sheet to LPA via email by 5/5/23. All other deficiencies cited on the 2/15/23 report have been corrected.

An exit interview was conducted with the director and appeal rights (LIC 9058 1/16) were discussed. The director’s signature on this form acknowledges receipt of these rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed the director post notice of site visit.

Based on observation, the licensee did not comply with the section cited above. LPA observed infant food for child #1 (C1) in the refrigerator labeled with the child's name but without the current date. This poses a potential health, safety or personal rights risk to children in care.
