On 03/01/2023 at 12:15pm, Licensing Program Analyst (LPA) Selina Siao conduct an annual inspection. Upon arrival, LPA met with Director Breeanna Mota and proceeded to tour the facility. There were 54 children in care. The following ratios were observed Preschool 1 class had 10 children eating lunch supervised by teacher Dayoung Kim, Preschool 2 had 11 children that just finished eating lunch and are getting ready to nap supervised by teacher Lisa Santos and teacher aide Christine Heidemann. Pre-K 2 class had 9 children supervised by teacher Lauren Ballard. Pathways class had 11 children napping on cots supervised by teacher Pratibha Singh. Early Preschool 1 class had 13 children napping on cots and were supervised by teacher Marwa Milhem and teacher aide Sarah Thompson. Appropriate ratio was observed during the tour. All required notices, forms and licenses were posted. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and drinking water are available for the children. Storage cubbies and backpack hooks are available for the children. Napping equipment consists of cots which are kept in classroom and each cot has its own sheet or covering. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a kitchen which is clean and free of hazards. Menu is posted. Adequate food is available for meals and snacks. Cleaning supplies are kept out of reach of children. Outdoor play area is a fenced playground with sufficient rubber for cushioning. Climbing structures and slides are securely fixed to the ground. Area has a cover patio area used for shade. Equipment is age appropriate and separated by age groups, infant and preschool. The children has there own water bottles that they use during indoor and outdoor activities. Grounds are free of debris or potential hazards. LPA reviewed first aid supplies and reviewed medication policy and storage, all areas are within compliance. Facility uses a digital signed in and out system. Isolation area is the Director's office.

Personnel and client records were reviewed, and staff members have the required immunizations and completed the online mandated reporter training. Reporting requirements was also reviewed. All personnel have required criminal record and child abuse index clearances or exemptions. LPA reviewed Emergency Disaster Plan and last fire drill was conducted on 02/15/2023. Facility has an operating carbon monoxide detector located in the kitchen. The opening and closing staff members has a valid EMSA approved CPR/FA when children are in care.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following items were discussed with Director: Director was reminded that corporal punishment and smoking are not allowed in day care. Director was provided with the effects of lead exposure information.

Child Care Providers can now sign up for Quarterly Updates and PINS through the DSS website at https://cdss.ca.gov/inforesources/community-care-licensing/subscribe. LPA discussed California Megan's Law with Director and advised her to go on the website at www.meganslaw.ca.gov.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

All unusual incident reports shall be submitted to Licensing office via email at SDIncidentReports@dss.ca.gov or via fax at (619)767-2203.

A notice of site visit was given and must remain posted for 30 days. Failure to keep notice posted will result in a civil penalty of $100.00.
