13 | On October 15, 2025, at 9:00 AM, Licensing Program Analysts (LPAs) Annette Sutherland and Nancy Diaz arrived at the facility to deliver findings related to the above-referenced allegations. Upon arrival, LPAs met with Director Erica Castaneda and conducted a tour of the facility.
During the investigation, LPA interviewed parents and current and former staff members. LPA reviewed children’s records and relevant documentation, including “ouch” reports. The information obtained shows that Child #1 (C1) has been repeatedly touching, tapping, and hitting other children in the classroom. These incidents have not been consistently documented by staff as they occur, despite them being ongoing throughout the day. Instead, the behaviors have only been communicated after children began exhibiting signs of emotional distress due to the repeated unwelcome interactions. Although the classroom is maintaining required minimum ratios with occasional extra support from rotating staff, this level of staffing is not sufficient to meet the needs of both C1 and the other children in the classroom. |
