On 6/17/25 at 2:05PM, Licensing Program Analyst (LPA) Annette Sutherland conducted an unannounced complaint inspection to deliver findings regarding the above allegations. LPA Sutherland met with Site Director Erica Castaneda and toured the facility. During the investigation, interviews were conducted with multiple parties. Facility observation was conducted, and relevant documentation was reviewed. Facility ouch report incidents were reviewed. Facility Staff reports incidents that are witnessed. If staff does not witness the incident occur but sees a mark, staff will report that to the parent. The allegations are therefore determined to be Unsubstantiated. A finding of Unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violation occurred. An exit interview was conducted with the Licensee. A Notice of Site Visit (LIC9213) and Appeal Rights (LIC9058) were provided along with the report (LIC9099) . LPA Sutherland observed Notice of Site Visit being posted. Notice of Site Visit must be posted for 30 days.
