On 3/6/25 at 10:50 am Licensing Program Analyst (LPA), Annette Sutherland, conducted a case management visit for the purpose of following up on 2 self reported incidents that occurred on 2/5/25 & 2/20/25. LPA met with Assistant Director Kayla Mendoza and toured the facility. Director Erica Castaneda arrived shortly. A child (C1) was running down the playground ramp and took to steps and tripped. C1 landed on the grass while C1 bumped their head and a slight cut on their forehead due to hitting the edge of the plastic water bin. First aid was provided from staff. Facility has since removed the water bin and replaced it with a softer plastic waterbin. 2nd incident was when child (C2) was climbing the outdoor wooden structure in the exploration area. C2 tripped on their closing and fell backwards. C2 was able to catch their fall. Staff (S1) witnessed C1 take the fall and provided first aid care at facility.


LPA toured outdoor exploration area where the incident occurred. At the time of the incident the program was within staff to child ratio requirements. There is always a staff member supervising the outdoor wooden structure. The playground has sufficient padding and is age appropriate. Based on today’s visit, no deficiencies were observed at this time. An exit interview was conducted, and this report was reviewed with Director Erica Castaneda

A notice of site visit was provided and advised that it must remain posted for 30 days.
