13 | Licensing Program Analyst (LPA) Jennifer Lott conducted a complaint investigation visit to deliver findings for the above allegation. LPA was greeted at the front door by Director, Felicia Early and was granted entry after identifying herself and disclosing the reason for her visit.
The Department’s investigation consisted of a review of facility and outside source records, interviews with staff and outside sources. It is alleged that on or about May 17, 2022, staff did not provide a safe and healthful environment resulting in a child sustaining injury while in care. Interviews with staff revealed that child #1(C1) had been playing on an outdoor play structure which the manufacturer states is for ages 2 to 5 years old. There were 2 staff assigned to the play structure, however as 1 staff left for break, neither the staff leaving for break, the new staff coming on duty nor the existing staff visually observed C1 climbnig the play structure ladder. C1 climbed up the ladder unnoticed, reached the top and was unable to stretch across to the platform due to their size. C1 fell 5 feet to the ground, (which was made of cushioned material) and sustained injury. |
