On 11/2/22 at 10:40am, LPA Patrick Ma and Selina Siao made an unannounced Plan of Correction visit for the purpose of clearing the deficiencies previously cited on 10/12/22 and 10/24/22 under Heath and Safety section 1596.955(a)(3). At arrival, LPA's met with Licensee/Preschool teacher Pam Thomsen and later met with Director Nicolle Daniels upstairs. A tour of facility was conducted. There were 11 children in room 1A with Pam Thomsen. There were 23 children in room 1B with staff Amy King and Lauren Digiacinto. There were 12 children in room 1D, designated for toddlers, with staff Alison Andrade Lopez and Monica Guzetta.

Facility was in ratio deficiencies cleared.

Facility was previously cited under section 101217(a) Personnel Records and is reminded the POC is due by 11/12/22.

Exit interview conducted and report was reviewed with the facility representative Nicolle Daniels. A notice of site visit was given and must remain posted for 30 days.
