On 12/13/24 at 11:00 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management inspection to follow up on an unusual incident reported by the facility on 9/24/24. Upon arrival, LPA met with Director Lauren Patlan who recently returned from leave and is now back at the facility since 12/3/24. Director stated she notified Licensing via duty line voice mail of her return to the position.

LPA observed a total of 28 children in the following classrooms:
  • Room 1 (Pre-K) (was outside on the playground) had 9 children with staff members Crystal Dodge and Morgan Pickett
  • Room 2 had 10 children with staff members Margarita Tablas & Elizabeth Carpio
  • Room 3 had 9 children with staff members Isabelle Becerra, Luz Martin and Nancy Morales (sub)

All staff were fingerprint cleared and associated to the facility. Director had proof that the incident report was submitted to: SDIncidentreports@dss.ca.gov on 10/1/24. According to the report, C1 had unexplained injuries that occurred during naptime in the classroom. A substitute (S1) was also present during the incident in the classroom, but did not return to the facility after the incident on 9/24/24. On 12/13/24, LPA spoke with Director and two of the staff members that were present during the incident with S1. Per staff members interviewed, S1 was not left alone with the children. On the day of the incident (9/24/24) there were 9 children in Room #1 (Pre-K). Supervision was in place and there was not a preponderance of evidence to suggest that the injuries were caused by S1.

No deficiencies were cited. Exit interview conducted and report was reviewed with facility representative Director Lauren Patlan. Notice of site visit was provided and must be posted for 30 days.
